This privacy policy explains the basis on which we collect, process and store your information and when we are legally and professionally required to disclose details. In this policy, “we”, “our” or “us” refers to Activ8 Physiotherapy.

Activ8 Physiotherapy acts as a Chartered Physiotherapist and Veterinary Physiotherapist.

For the purposes of the Data Protection Act 1998 and the General Data Protection Regulation (GDPR) 2018 the Data Controller is:

What information will be stored?

·      Information that you provide us with in order for us to contact you e.g. name, address, telephone number, email address, address of where your horse is stabled,

·      Clinically relevant information about you or your horse as provided by your doctor or the referring veterinary surgeon.

·      Clinical details (e.g. notes and reports) pertaining to any care or treatment your horse has received from us.

·      Details of any correspondence we have had with you e.g. enquiries, quotes.

·      Any client feedback you provide.

·      Payment related information that you provide to us when payment is received for our services.

 

How data will be collected

We may collect data from you if you contact us through the website or Facebook page, via email, telephone or by post, thus directly from you. Please note that contacting us through social media or electronically is not an entirely secure method of transmission, therefore if you chose to contact us via these methods you do so at your own risk.

We may also collect data via your doctor or referring veterinary surgeon in order for us to arrange an appointment with you.

 

How do we use your information?

Our lawful basis for processing your data is Legitimate Interests. We use the information we collect from you in a business operational capacity to provide you with the veterinary services that you have requested, to provide you with information by post, email, telephone or otherwise about those services and to notify you about changes to our services. We also process personal information to maintain our accounts and records. We may also use your information to notify you about news, events and other updates we consider may be of interest to you where you have opted in to receive such communications.

 

Where we store your Personal Data

We will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy. Any personal or sensitive data recorded on paper will be stored in a lockable cabinet. Personal and sensitive data stored electronically are password protected. Only those authorised and trained will be able to access your information. As required by law records will be kept for a maximum of 7 years following the last appointment. Once the retention period has expired, all records will be securely destroyed.

 

Disclosure of your information

Under no circumstances will any personal information be shared without your consent. We will never sell, trade, or give your personal information to others. However, we may share it with governmental, judicial or fraud prevention agencies to comply with our legal and regulatory obligations.

Relevant clinical information may be passed to and from your veterinary surgeon (in line with the Veterinary Surgeons Act 1966) in order to provide the best care for your horse.

 

Your right to your information

You have the right to formally request in writing to receive a description of personal data stored, the purposes for which data is being processed, who is receiving personal data and how long data will be retained. Patients may also enquire about their rights of rectification, erasure, restriction and objections.

You may also make a formal request in writing to receive a copy of their personal and sensitive data. In accordance with the GDPR the requested data must be provided within 1 month at no fee (unless repeated or unreasonable requests are made).
