Veterinary Consent Form

Equine physio services in Suffolk, North Essex and South Norfolk

The latest RCVS regulations state that veterinary consent is no longer required for general check-ups or maintenance physiotherapy.

To carry out treatment on any horse with ongoing veterinary issues or those who have had recent veterinary treatment (other than routine input), I will need consent from your vet prior to the appointment. This is so physiotherapy can be provided in a timely manner and note interfere with any veterinary treatment.

Please use the buttons below to download the Activ8 Physiotherapy Veterinary Consent Form which may need to be signed by your vet.

  • "Ella was referred to us by my vet after a horse of mine had a bad accident three years ago. Since then she has become a major part of our team, looking after my former European Pony to now my Junior Gold medalist and my other two horses. She comes at least twice a month and not only do the horses love her, she really does help them preform to their best. Ella works alongside my vet and we all try to leave no ‘stone’ unturned. We feel very lucky to have Ella Gay as a member of our team."
